Hoi Kees,
In the past I used Forms To Go from Bebosoft a lot.
windowsbased tool that generates a html/php/sql form script.
Very handy and easy to use.

The questions was a form builder for a standalone site. Not wordpress websites.
Gravity is one of the best wordpress plugins for managing forms.
Only not suitable for standalone websites.

I use CoffeeCup web form builder. Kind of simple but it is actively developed. fairly cheap and easy to work with. They just added conditional options to it. Buy it once and you get updates forever.
The developers claim they are working on some type of responsive integration. That will make it a real contender.
Nick had pointed out that Google Drive has a form builder but it's kind of limited and only is good for that account. In other words no multi email function.
