Hi there
I am using a trial version and before I buy I would like to resolve the header issue I am having
I am creating my header in Photoshop. I cannot find information on what dimensions and format I should be creating my header in?
When I do import an image as a header into Artisteer it is terribly deformed. Bad quality. I do not have any transparancy or glare settings enabled.

You can have your header any size you wish. Import it as png or jpeg.
Once done, click on the "position" icon beside where you choose to import the header image from the toolbar, then you can choose the correct size/position.
You might need to then resize the header to fit your image.
